1. Acknowledgment of Inherent Risks
I understand that participation in Brazilian Jiu Jitsu (BJJ), martial arts training, and related physical activities involves inherent and significant risks. These include, but are not limited to: cuts, bruises, sprains, fractures, joint injuries, dislocations, concussions, paralysis, and death.
I understand that BJJ includes close physical contact, grappling, submissions (including chokeholds and joint locks), and live sparring (“rolling”). Injuries may result from my actions, the actions of others, or the condition of the training environment.
I knowingly and voluntarily assume all such risks, both known and unknown.
2. Medical Fitness & Responsibility
I certify that I am physically and mentally fit to participate. I do not have any condition that would prevent safe participation, or I have disclosed it below:

- Stop training immediately if I experience pain, dizziness, or injury
- Inform the instructor of any injuries or limitations
- Seek medical attention if necessary
3. Communicable Disease Acknowledgment
I understand that participation in Brazilian Jiu Jitsu and related activities may expose me to communicable illnesses and infections, including but not limited to bacterial, viral, fungal, and skin infections such as MRSA, staph infections, ringworm, and other contagious conditions. I voluntarily assume all risks associated with such exposure.
4. Emergency Medical Authorization
In the event of injury, accident, or medical emergency during participation at PHD BJJ, I authorize PHD BJJ and its representatives to obtain emergency medical treatment on my behalf if deemed necessary. I understand that I am solely responsible for any medical expenses incurred.
5. Release of Liability (Including Negligence)
To the fullest extent permitted under Oregon law, I hereby release, waive, and discharge:
PHD Jiu Jitsu, its owner(s), instructors, training partners, guests, and any affiliated individuals from any and all liability, claims, demands, or causes of action arising out of or related to any injury, damage, or loss I may suffer as a result of participation.
This release includes claims based on negligence of the released parties, except where such liability cannot be waived under Oregon law.
6. Covenant Not to Sue
I agree that I will not bring any legal action or claim against PHD Jiu Jitsu or its representatives for injuries or damages arising from participation in training activities.
7. Rules & Safety Compliance
I agree to:
- Follow all instructions and safety guidelines
- Train in a controlled and respectful manner
- Tap early and release submissions immediately upon a tap
- Maintain proper hygiene and appropriate attire
- Refrain from training under the influence of drugs or alcohol
8. Home Gym Acknowledgment
I understand that training takes place in a private, home-based gym environment, which may not have the same safeguards as a commercial facility. I accept all risks associated with this setting.
9. Personal Property
I understand that PHD Jiu Jitsu is not responsible for lost, stolen, or damaged personal property.

11. Minor Participants
If the participant is under 18, a parent or legal guardian must sign. The parent/guardian agrees to all terms and assumes all risks on behalf of the minor.
12. Governing Law & Venue
This agreement shall be governed by the laws of the State of Oregon. Any legal action shall be brought exclusively in a court of competent jurisdiction within Oregon.
13. Severability
If any provision of this agreement is found to be unenforceable, the remaining provisions shall remain in full force and effect.
14. Acknowledgment & Voluntary Signature
I have carefully read this agreement, fully understand its contents, and understand that I am waiving certain legal rights, including the right to sue. I sign this agreement freely and voluntarily.
By signing below, I agree that this electronic signature is the legal equivalent of my handwritten signature and that I have read and agree to this Waiver and Release of Liability.
